The incumbent will serve in a developmental assignment to provide the individual with the knowledge, skills, and abilities needed to perform the full range of duties as described in the target position description. Health Coaches both enhance wellness and assist with management of chronic disease. Health Coaches may work in primary care as a member of a PACT to assist Veterans with weight management, smoking cessation, addressing problematic areas in the eight components of proactive health and wellbeing, and adherence to chronic disease such as diabetes, congestive heart failure and chronic pain. Health Coaches may work in specialty care-including mental health, nephrology, oncology, cardiology, infectious disease and others-to assist Veterans with identifying goals, developing behavior plans, and taking action steps to increase adherence to treatment. Health Coaches may work on inpatient units to assist with identifying goals, developing behavior plans for discharge, and assisting with post-discharge follow-up. In all settings, Health Coaches conduct interventions that lead to improved functioning, decreased probability of deterioration or admission, and enhanced quality of life. The following duties are performed: Contacts eligible participants for enrollment and explain the health coaching process. Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones. Conducts interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settings. Conducts Whole Health Facilitated Groups; participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other inpatient and outpatient groups as appropriate. Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments. Actively guides Veterans in the exploration of core elements of personal health planning through individual and group based programs. Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.
