Job Summary:
The Nurse Practitioner provides clinical services to the appropriate programs and its patients/in both the inpatient and outpatient setting. The Nurse Practitioner is actively involved in providing formal and informal care, education and participates in the review and development of specific policies, procedures, and practice guidelines. The incumbent in this position must be able to demonstrate effective interpersonal skills and an understanding of the interdependent roles of various allied health professions. This individual will assess physical, psychosocial, cognitive, and functional components, develop a treatment plan, order and interpret diagnostic components of the plan, and prescribe non-pharmacological and pharmacological agents.
The Nurse Practitioner functions professionally under a practice agreement with the supervising physician and operationally under the Vice President Nursing Services and/or the designee.
Essential Functions & Responsibilities:
- In collaboration with physician, demonstrates ability to consul patients and family members about diagnosis, testing, surgery and risk of procedures.
- Performs comprehensive health assessments for the purpose of diagnosing and treating individuals.
- Formulates appropriate differential diagnosis, develop treatment plan to include appropriate diagnostics; identifies non-pharmacological interventions, appropriate pharmacological agents and education. The plan is based on mutually established priorities to maximize the heath potential of the patient.
- Provides relevant patient and family education; makes appropriate referrals to other professional and community agencies.
- Orders and provides preliminary interpretation of diagnostic studies.
- Prescribes non-pharmacological therapies and pharmacological agents within the defined scope of service and according to guidelines established by the supervising physician.
- Demonstrates the ability to use effective organizational/prioritization problem-solving and decision-making abilities in practice.
- Provides emergency treatments and initiates emergency life saving measures per ACLS guidelines.
- Meets all the requirements of the practice agreement with the supervising physician under the regulations and requirements of the Commonwealth of Virginia.
Delineation of Privileges:
- Obtain, record and dictate medical histories
- Perform, record and dictate physical examinations
- Make daily rounds, record progress notes, write routine comfort and diet orders, order laboratory tests and non-invasive examinations, write approved routine orders, and change dressings
- Prepare patients for discharge from the hospital
- Dictate discharge and transfer summaries
- Evaluate patients who require a surgical consult
Clinical procedures:
- Apply and change dressings
- Remove sutures and staples
- Perform venipunctures
- Insert IV needles and catheters
- Catheterize urinary bladder
- Pass nasogastric tubes
- Place and remove chest tubes and monitoring lines
- Irrigate chest tubes and catheters
- Remove intra-aortic balloon pump
- Debride, irrigate and pack surgical wounds
- Remove drainage catheters
Communication:
- Educate patient/family regarding disease, procedures, medication, nutrition, healthcare, and post-discharge care
- Dictate letters to physicians, patients
- Act as liaison between patients and physicians
- Act as liaison between nursing and physicians
- Request consults from health agencies and act as liaison
- Make and receive phone calls to/from patients, physicians and nurses for information, advise, triage
Other
- Maintain statistics
- Complete insurance forms and home health forms
- Maintain written material; teach devices and special equipment for use of personnel and patients.
Qualifications:
- Registered Nurse licensed to practice in Virginia or eligible, required
- MSN, required
- Current Virginia state certification as an Adult Nurse Practitioner, required
- AHA BLS Provider CPR required
- Recent adult medical-surgical experience (5 years of experience), required
- Ability to work effectively in emergency situations and under stress
- Proficient leadership skills
- Ability to work cooperatively/collaboratively as part of a team, developing productive working relationships with diverse groups of people.
Additional Qualifications for Nurse Practitioner, Cardiology Team:
- Current Virginia state certification as an Adult Nurse Practitioner with preferred focus in adult cardiology.
- Recent adult medical surgical experience (5 years experience to include 2 years in cardiology specialty).
Additional Qualifications for Nurse Practitioner, Perinatology:
- Current Virginia state certification as a Nurse Practitioner with preferred focus in women's health.
- Recent ante-partum/labor & delivery experience (5 years experience) preferred.
Additional Qualifications for Nurse Practitioner, Lead in Observation Unit:
- Two years in Advanced Practice Nursing and at least 2 years of experience in-patient care, required.
Additional Qualifications for Nurse Practitioner, Structural Heart:
- Minimum of five (5) years of adult medical-surgical experience, preferred
- Minimum of five (5) years of experience in the management of cardiac patients, preferred
- Minimum of three (3) years of clinical experience in an acute hospital environment, preferred
- Exceptional communication skills (verbal and written)
- Proven proficiency IS technologies and software (i.e., Registry database, Excel, etc.)
Additional Duties for Nurse Practitioner, Perinatology:
Under the supervision of Perinatologists, the NP will:
- Evaluate and treat patients with gestational and pre-gestational diabetes mellitus according to current clinical guidelines and hospital or practice written guidelines
- Provide comprehensive pre-conceptional care to high risk patient populations, including counseling, ordering diagnostic /screening tests and ordering medications or titrating existing medications as indicated according to current clinical guidelines
- Provide postpartum care, blood glucose screening and counseling to patients with gestational diabetes and refer patients to appropriate care as indicated
- Provide patient education as well as in-service to nursing staff and physicians in the areas of latest clinical guidelines for diagnosing, treating, and managing patients with diabetes during pregnancy
- Serve as liaison between Perinatologists, Endocrinologists, OB-GYN, diabetes management program and other health care disciplines to provide holistic and integrated patient care and services
- Serve as a resource in diabetes self-management skills /education for Perinatologists and clinical staff
Additional Duties for Nurse Practitioner, Lead in Observation Unit:
- Participates with Leadership and Physicians in the recruitment, selection, orientation, and retention process. Serves as a resource for clinical team members.
- Provides coaching and mentoring as needed.
- Assures that Advanced Practice Practitioner (APP) services are available 24 hours/day, 7 days/week. This includes making assignments, monitoring productivity, consulting on difficult cases, and ensuring adequate coverage.
- Serves as a liaison between physicians, MWHC departments, patients, caregivers, and families to resolve concerns.
- Participates in QAPI program. Identifies opportunities for improving patient outcomes, participates in audits to monitor outcomes, and assists Associates in identifying and monitoring opportunities for quality and performance improvement.
Additional Duties for Nurse Practitioner, Structural Heart:
- Participates with Leadership and Physicians in the recruitment, selection, orientation, development, and retention process.
- Assures that Advanced Practice Practitioner (APP) services are available 24 hours/day, 7 days/week. This includes, but is not limited to creating assignments, monitoring productivity, consulting on difficult cases, and ensuring adequate coverage.
- Participates in Quality Assurance and Performance Improvement (QAPI) programs by identifying opportunities for improving patient outcomes, participating in audits to monitor outcomes, and assisting Associates in identifying and monitoring opportunities for quality and performance improvement.
- Serves as a clinical resource for identification, tracking and treatment alternatives for structural heart patients; recommends diagnostic studies and/or clinical interventions when appropriate, as well as measures patient-specific and aggregate outcomes; tracks new technology trends, research findings, treatment models, etc.
- Collaborates with medical leadership to incorporate new technologies, treatments, and research into clinical practice.
- Tracks and analyzes physicians' practices, referral patterns and market data; recommends initiatives in response to community need and growth opportunities.
- Works with physician leader(s) to prepare clinical in-services, CME courses, marketing, and collateral materials to create program awareness and understanding. Routinely reviews and ensures patient and referring physician education and collateral materials are current, coordinated, available and effective.
- Partners with physician and hospital leaders for the Structural Heart Center to identify and recognize partnership(s) and growth opportunities.
- Interfaces with industry representative(s) for program awareness, research and partnership opportunities.
- Works with physician leaders for program content and structure. Builds internal and external program awareness, development and growth.
- Coordinates monthly valve conference.
- Assists with maintaining registry/database (such as transcatheter aortic valve replacement (TAVR) and left atrial appendage occlusion (LAAO) registry) consistent with the recommendations and guidelines set forth by Society of Thoracic Surgeons / America College of Cardiology Foundation (STS/ACCF); provides outcome reports from the transcatheter valve therapy (TVT) registry to the administrative team.
