The primary purpose of this position is to provide direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department while assigned as a Patient Aligned Care Team (PACT) Nurse Care Manager (NCM) at the Mann-Grandstaff VA Medical Center in Spokane, Washington or the Community Based Outpatient Clinic in Spokane Valley, Washington.
