Overview

St. Luke's Health System is currently seeking a full-time Nurse Practitioner to join the Home and Community Based Services team. This provider will partner and house with St. Luke's Home Health and Hospice teams ( Meridian location), servicing our Treasure Valley area.

We are looking for a practitioner with experience in either Internal Medicine, Family Medicine, Emergency Medicine, Urgent Care, or other high-volume clinic managing chronic illness.

Home health or hospice background is preferred.

This provider will work collaboratively with the home care team and the community primary care provider (PCP). They will be responsible for supporting the home care team when timely intervention is required. They will travel to patient homes and perform billed provider visits, providing safe care in the home when the patient is unable to travel timely to a clinic appointment. Typical patients situations managed include chronic diseases such as CHF, COPD, and diabetes. In addition to partnering with the patient's PCP or specialist, the NP will work closely under the sponsorship of a designated physician.

This position will include a small on-call requirement in a rotation with other providers (holiday coverage). The primary schedule is regular weekday (Mon- Friday 0830- 5). Full time.

JOB REQUIREMENTS INCLUDE:

• Graduation from an accredited school with a minimum of a Master's Degree and certification, as required by the specialty, as well as eligibility to obtain licensure from Idaho to practice as a Nurse Practitioner
• Must obtain and maintain unrestricted prescriptive authority, DEA, and controlled substance licenses.
• Excellent communication skills to include oral and written comprehension/expression.
• Ability to manage a fast-paced, busy clinical environment related to changing patient needs, including work with patients with acute, chronic, and complex disease processes.
• Ability and willingness to exhibit behaviors consistent with standards for performance improvement and organizational values (e.g., efficiency & financial responsibility, safety, partnership & service, teamwork, compassion, integrity, and trust & respect).
