This position plans and provides primary and clinical pharmacy care in Ambulatory Care and Endocrinology areas at Midtown CBOC. The primary functions are managing therapy of patients with chronic medical conditions to achieve predetermined therapeutic goals; responding to consult requests from medical staff; improving success rate on critical medical center performance measures; educating and training medical and allied health care staff on medication therapy and disease management guidelines. Diabetes Technology (1) Functions under a scope of practice to manage patients on insulin pumps in collaboration with Endocrinology providers. (2) Oversees continuous glucose monitor updates, processes, supplies, and order sets at the Phoenix VA Health Care System. (3) Point of contact for diabetic software platforms and liaison between company representatives to troubleshoot technology issues.
