This position is responsible for the delivery of Whole Health Coaching and skill development of the Michael E. DeBakey VA Medical Center's Whole Health Program, including provision of care through coaching and promotion and education regarding Whole Health Coaching to Veterans and staff. To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 02/03/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-05. For a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. Individual Occupational Requirements Basic Requirements: The following education or experience are required to meet the specific qualification requirements for this position. Degree: behavioral or social science; or related disciplines appropriate to the position. OR Combination of education and experience that provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. Four years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. You may qualify based on your experience and/or education is described below: GS-07 Qualification: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Applying knowledge of regulations, policies, and procedures, regarding centralized information systems such as Scheduling and Enrollment, Health Data Repository, and local policies; and participating in outreach programs to inform, in this case, Veterans and family members of available programs. To qualify based on education for this grade level you must have successfully completed 1 year of graduate-level education or superior Academic achievement in a field directly related to the position to be filled. GS-09 Qualification: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization. Physical Requirements: Regular and recurring work is mostly sedentary involving sitting at a desk, although the position requires the ability to move from one geographic location of the facility to another. Work is performed in an office setting with occasional visits to areas within the service as well as throughout the Medical Center. Work area is adequately lighted, heated and air conditioned. ["As a Social Science Program Coordinator, you will: Contact eligible participants for enrollment and explain the health coaching process. Provide health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones. Conduct interventions for Veterans using one-on-one, couple, group modalities in outpatient and inpatient settings. Conducts Whole Health Facilitated Groups; participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other inpatient and outpatient groups as appropriate. Review current health behaviors and develops customized treatment plans with Veterans. Coordinate care with Veterans' treatment providers, both primary and specialty care, to ensure identified goals are consistent with best practices for treatment and management of chronic illness. Use VISTA, CPRS, Microsoft office software, Intranet and Internet and other applicable software to track and support patient progress. Provide educational opportunities for staff-including lunch and learns, grand rounds, and presentations at staff meetings-in order to promote health coaching and encourage referrals to these services. Assist with data management analyses, special studies, and reports for use by management for budget preparation, workload projections, and quality assurance and utilization reviews. Provides training and troubleshooting for other health coaches in health coaching administrative processes and associated supporting issues. All other duties assigned by management. Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met.
