This position is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching and promotion and education regarding Whole Health Coaching to Veterans and staff. To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 08/18/2022. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-5. The grade may have been in any occupation, but must have been held in the Federal service. For a GS-09 position you must have served 52 weeks at the GS-7. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Candidates must meet the following Individual Occupational Requirement (IOR): Degree: behavioral or social science; or related disciplines appropriate to the position. or Combination of education and experience that provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. or Four years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. You may qualify based on your experience and/or education as described below: Specialized Experience GS-07: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: knowledge of health coaching structure, process, skills, and techniques. Knowledge of pertinent laws, regulations, policies, and precedents to determine their impact on the customer service program and related support resources. Operational knowledge of VA rules and regulations regarding eligibility, enrollment, income assessment, and insurance for veterans' medical care to provide educational guidance to the Veteran, knowledge of VA inpatient and outpatient programs and operations, admissions, scheduling, and knowledge of laws and regulations relating to the protection of the privacy of medical information. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have 1 year of graduate-level education or superior academic achievement. Operational knowledge of VA rules and regulations regarding eligibility, enrollment, income assessment, and insurance for veterans' medical care to provide educational guidance to the Veteran, knowledge of VA inpatient and outpatient programs and operations, admissions, scheduling, sharing agreements, and travel as well as operational knowledge of laws and regulations relating to the protection of the privacy of medical information. To qualify based on education for this grade level you must have 2 years of progressively higher-level graduate education leading to a master's degree or master's or equivalent graduate degree. ["Health Coaches both enhance wellness and assist with management of chronic disease. Health Coaches may work in primary care as a member of a PACT to assist Veterans with weight management, smoking cessation, addressing problematic areas in the eight components of proactive health and wellbeing, and adherence to chronic disease such as diabetes, congestive heart failure and chronic pain. Health Coaches may work in specialty care-including mental health, nephrology, oncology, cardiology, infectious disease and others-to assist Veterans with identifying goals, developing behavior plans, and taking action steps to increase adherence to treatment. Health Coaches may work on inpatient units to assist with identifying goals, developing behavior plans for discharge, and assisting with post-discharge follow-up. In all settings, Health Coaches conduct interventions that lead to improved functioning, decreased probability of deterioration or admission, and enhanced quality of life. Health Coach:\nContacts eligible participants for enrollment and explain the health coaching process.\nProvides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones.\nConducts interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settings.\nConducts Whole Health Facilitated Groups; participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other inpatient and outpatient groups as appropriate.\nProvides follow-up care through telephone clinics or secure messaging between face-to-face appointments.\nDevelops treatment plans for Veterans focused on behavioral interventions based on current scientific knowledge and research that improve quality of life and reduce risk for chronic disease, or that reverse or slow progression of chronic disease.\nUtilizes motivational interviewing and health coaching techniques and processes, learned through prerequisite health coach training, to assist members in developing a realistic plan to accomplish their goals. Program Development\nDevelops information for Veterans about the Whole Health Group and Individual Programs. Conducts outreach activities to inform Veterans and family members of available programs.\nMeets with external stakeholders to provide information about the Whole Health Group and individual programs and access to Veterans.\nDevelops and conducts onsite educational series for enrolled Veterans, family members, and VA employees to explain health coaching, including its uses and benefits.\nAssists with data management analyses, special studies, and reports for use by management for budget preparation, workload projections, and quality assurance and utilization reviews.\nParticipates in the establishment of administrative standards locally; the objective of which is intended to improve programs, facilities and operations.
