DescriptionSummary:The Coordinator Diabetes Education Lead is responsible for the delivery of patient care education through the process of assessment, diagnosis, planning, implementation and evaluation. In addition, the Diabetes Coordinator works closely with the Director of Rehabilitation and oversees the planning, implementation, and evaluation of the Outpatient Diabetes Self Management Education (DSME) entity. Coordinates diabetes education and medical nutrition therapy with other disciplines and collaborates to provide quality patient education. Assists in the follow up patients.
