The Fayetteville VAMC is looking for an RN to serve as the Certified Diabetic Educator. The CDE uses the nursing process and evidence-based practice to collaborate with the Patient-Aligned Care Team (Patient, Primary Care Provider, RN Care Manager, Health Technician/LPN and clerical staff) and expanded team members (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient centered holistic care plan.
