The Optum Dual Special Needs Plan (DSNP) at Home program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in their place of residence. The DSNP program combines Optum trained clinicians providing intensive interventions customized to the needs of each individual, in collaboration with the Interdisciplinary Care Team, which includes the Optum clinician, the member’s Primary Care Provider and other providers, and other professionals. Optum providers serve people in their own homes through annual evaluations, ongoing visits for higher risk members, care coordination during transitions from the hospital or nursing home and ongoing care management.
As a Telephonic RN Case Manager with Optum at Home, you'll support a diverse member population with education, advocacy, and connections to the resources they need to feel better and get well. Instead of seeing a handful of patients each day, your work may affect millions for years to come.

Work Schedule: Monday-Friday, 8 am-5 pm in the market that you are assigned.

Primary Responsibilities:
- Make outbound calls and receive inbound calls to assess members' current health status
- The RPM RN will monitor the Vivify system for alerts created by enrolled members who are communicating health information that may indicate a potential change in condition
- Maintain access to Vivify Care Team Portal throughout the day to monitor alerts for enrolled members. Provide patient education to assist with self-management of chronic health conditions
- Prompt outreach to member to triage alerts for signs of worsening condition and need to escalate to appropriate medical team (i.e., APC, PCP, or specialist)
- Provide a complete continuum of quality care through close communication with members on-phone interaction
- Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels
This is high volume, customer service environment. You'll need to be efficient, productive, and thorough dealing with our members over the phone. Strong computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes.

Required Qualifications:
- Associate degree or higher in Nursing (RN)
- Current unrestricted RN License in state of residence
- Unrestricted Compact RN License and may need to apply for additional licenses as needed
- Unrestricted Washington State RN License
- 2+ years case management experience
- Medicare and/or Medicaid managed care experience
- Experience educating members regarding chronic conditions
- Able to type and have the ability to navigate a Windows based environment and multiple platforms
- Access to High-Speed Internet (Cable or DSL)
- Dedicated workspace from home
- Willing to conduct Video conference calls with members

Colorado, Connecticut or Nevada Residents Only: The salary range for Colorado residents is $54,400 to $97,000. The salary range for Connecticut / Nevada residents is $60,000 to $106,700.
