The Clinical Pharmacy Specialist (CPS) - Home-Based Primary Care (HBPC) is responsible for providing clinical pharmacy services in the Home-Based Primary Care Program at VA Southern Nevada Healthcare System (VASNHS). This may include rounding in veteran's homes, outpatient clinics and or the inpatient setting, participation on various committees pertaining to HBPC, reviewing charts and drug therapy for patients, performing clinical assessments and interpreting lab data. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Grade Determinations: GS-13 Clinical Pharmacy Specialist : Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. VHA Health Professions Trainee (HPT): VHA HPTs pending the completion of education or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. An HPT is an individual appointed to a temporary position in one or more VHA medical facilities performing clinical or research training experiences to satisfy program or degree requirements or receiving VA Health Professional Scholarship Program. Physical Requirements: The work requires standing, light carrying and lifting (under 15 pounds); walking, bending, reaching above the shoulder, and ability to distinguish basic colors. The incumbent may be involved in research activities also providing drug information services including those services requiring extensive interpretation, providing pharmacokinetic consults as clinically privileged, and providing in-service educational presentations to medical center staff. Extensive documentation and clinical progress note writing are required for this position. The incumbent may be assigned duties in other areas of clinical pharmacy as a secondary function. Additionally, this pharmacist is responsible for providing clinical services for chronic disease state management not limited to: dyslipidemia, hypertension, diabetes, congestive heart failure, chronic obstructive pulmonary disease, tobacco cessation, osteoporosis, hepatitis C, pain management. The CPS is also responsible for providing clinical services for the Anticoagulation Clinic through coordination and collaboration with the Anticoagulation Clinic. Clinical Pharmacy Specialists are employees of their medical center's Pharmacy Service, to ensure they receive all professional guidance, policy and communications, along with department missions and goals, especially regarding the VA National Formulary, VISN and local medical center Pharmacy and Therapeutic (P&T) Committee objectives for formulary management activities and patient safety. The Clinical Pharmacy Specialist plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. Incumbent is required to maintain current on professional licensure, Basic Life Support (BLS) Certification and VHA privacy policy trainings as per VA employment standards. Clinical Practice\n- Maintains a clinical practice through provision of patient care during new patient and/or follow-up visits. Coordinates care for patients with other healthcare providers.\n- Responsible for all HBPC patients registered in the program, must work independently and effectively to assure appropriate, accurate drug reviews are completed; interpret prescriptions and/or physician orders for accuracy, eligibility, availability, cost effectiveness and therapeutic appropriateness; provide and coordinate medication management services according to a broad scope of practice which includes reviewing patient medication profiles, contacting physicians, making authorized changes in orders or prescriptions as required, and ordering appropriate lab tests.\n- Accountable for medication reconciliation according to procedure.\n- Functions independently and in collaboration with an interdisciplinary team in day-to-day clinical and administrative duties associated with the position, consulting with higher level supervisors only when required. In general, uses a high degree of appropriate professional judgment to solve complex problems, which routinely arise throughout the workday.\n- Demonstrates recognized expertise in the area of geriatrics and palliative care the HBPC Pharmacy program. The CPS interacts with physicians and other health care professionals as demonstrated by regular attendance at HBPC multidisciplinary team meetings, medication review rounds, and other meetings for the purpose of providing consultation in all Pharmacy related matters.\n- Works closely with physicians and nurses to monitor patient response to medications, including efficacy, safety, and outcome, with special attention to polypharmacy problems in the elderly.
