This pharmacist functions as a Pharmacy Supervisor for a specific section of VAPAHCS Pharmacy Service. The Pharmacy Supervisor performs administrative functions independently and is responsible for all clinical work performed within the section. During off-tours, the Pharmacy Supervisor is responsible for making decisions during urgent/emergency situations which cannot wait until the next business day in order to ensure continued operations and timely patient care. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Grandfathering Provision. All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. Physical Requirements. See VA Directive and Handbook 5019. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Preferred Experience: Preferred Experience: Experience working as a clinical pharmacy specialist providing direct patient care in a primary care or specialty clinic; or completion of a PGY-1 residency, PGY-2 Ambulatory Care or Health System Administration, or has previous supervisory experience. Grade Determinations: GS-13 Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. Pharmacy Supervisor: Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. Pharmacists assigned to this position must demonstrate the following KSAs: a. Ability to effectively supervise subordinate staff. b. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. c. Expert understanding of regulatory and quality standards pertaining to pharmacy. d. Skill in managing people or programs. VHA Health Professions Trainee (HPT): VHA HPTs pending the completion of education or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. ["VAPAHCS is a 1000 bed multi-division Health Care System with specialties in Medicine, Surgery, Intensive Care, Psychiatry, Spinal Cord Injury, Sub Acute, Hospice, Nursing Home Care, and Poly-trauma Medicine and provides a full complement of inpatient and outpatient pharmacy services and programs. VAPAHCS is a teaching facility affiliated with Stanford University, School of Medicine, and University of the Pacific, School of Pharmacy. The VA Palo Alto Health Care System has immediate openings for TWO full-time Clinical Primary Care Pharmacy Supervisors/Clinical Pharmacy Specialists. Given expansion and growth in the Primary Care pharmacy section, we have converted one supervisor position into two positions; to allow for less direct reports and maintenance of a clinical practice. The primary function of this position is to provide oversight of the clinical ambulatory care section across the health care system. This includes personnel management of 12.0 -15.0 FTEE Ambulatory Care Clinical Pharmacy Specialists (CPS) each (1.0 Clinical Pharmacy Technician) located throughout seven (7) clinic geographic locations, as well as ambulatory care pharmacy program management. The incumbents report directly to the Associate Chief, Primary Care & Specialty Clinics, Pharmacy Service. The incumbents are responsible for planning, directing and coordinating the ambulatory care clinical program associated with providing independent practitioner services in Patient Aligned Care Teams (PACT), Anticoagulation, and Home Based Primary Care (HBPC). This includes a broad spectrum of disease state management under individual pharmacist scopes of practice including, but not limited to; diabetes, hypertension, hyperlipidemia, anticoagulation, heart failure, COPD, pain and geriatric deprescribing. The incumbents will maintain a clinical practice one to two days per week, in a PACT or specialty clinic utilizing their own independent scope of practice. Specific responsibilities include, but are not limited to the following: Providing direct supervision of employees; including performance and personnel management. One position will supervise all PACT, HBPC, Renal, Telehealth CPSs at the Palo Alto, San Jose, and Monterey clinics [14.7 FTEE]. One position will supervise all PACT CPSs servicing the Livermore, Fremont, Sonora, Stockton and Modesto clinics and the centralized Anticoagulation CPSs and Clinical Technician [12.5 FTEE]. Conducting process improvement and/or quality assurance projects to assess program performance based on national, VISN and local performance measures. Developing long range program planning and development for ongoing program relevance and growth, assuring implementation, maintenance and compliance with VHA guidelines and standards of professional practice. These supervisors serve as subject matter experts within Pharmacy Service and with other facility health care staff and all levels of hospital management regarding ambulatory care clinical services.
