This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Learn more The Licensed Practical Nurse (LPN) at the full performance level is technically proficient and self-directed in initiating, performing and completing complex practical nursing care to patients in the LPN PACT Ambulatory Care COVID Clinic. The LPN GS-7 must demonstrate an exceptional degree of practical nursing knowledge and skills necessary for concentrated effort and self-direction in carrying out the most complex practical nursing care assignments within the Outpatient setting. The LPN exhibits independent technical judgment, critical thinking skills, and the use of independent judgment to carry out complex practices and procedures. The LPN demonstrates knowledge, experience, skill and self-motivation to carry out complex patient assignments, which include prioritization and coordination of clinical activities with minimal supervision. The LPN has independent duties within team to ensure that the patients are receiving high quality care and have timely access to care. The LPN collaborates with a Specialty team to provide complex/specialized patient-centered care to patients' ages, cultural backgrounds, education level, and any other special needs. The LPN works under the direct supervision of a Registered Nurse and/or MD/DO, PA/NP in insuring care to veterans receiving specialty procedures. Possesses the knowledge and skill to carry out more specialized patient care activities. This knowledge base includes the use of patient data and the ability to navigate databases, such as VSSC. The Drive Thru LPN utilizes patient data to assist in care within the Drive thru as well as CBOC's and outpatient Lab.. The Ambulatory Care LPN uses Primary Care Almanac and PACT Compass as resources to identify and target chronic disease indicators and provides population management in collaboration with the PACT Team. The LPN initiates action to improve Veterans' overall health through use of population management data such as: Diabetic Cohort List Hypertension Cohort List Diabetic Outlier List Opioid Therapy Risk Report Drive Thru appointment management list Outpatient Lab appointment list Performs direct patient care activities, as appropriate to assignment, (includes, but is not limited to): Manages patient care problems and deviations using self-direction in accordance with VA Nursing Practice and LPN licensure standards. Works directly in the COVID Drive Thru assessing needs, signing in, and assist with navigation of veterans to eligibility. Collection of COVID specimens in drive thru and Phlebotomy as needed in the outpatient lab. Administration of vaccines in the Drive Thru. Assesses, records and reports vital signs, neurological signs, weight, and blood glucose levels. Appropriately triages and prioritizes patient needs as routine, urgent, or emergent in the clinic setting and modifies patient care/treatment as needed. Educates and documents patient and caregiver in the home management of wounds. Checks patient into the clinic, interviews patient and documents intake data as per policy.
