M Health Fairview is hiring a Clinical Pharmacy Lead at our Fairview Southdale location in Edina, Minnesota. Our pharmacy department serves a large and diverse patient population. This role will primarly be located at Fairview Southdale, but will partner with colleagues throughtout the system to drive initiatives for several key services. Anticipated areas of coverage include: Antimicrobial Stewardship, Student rotation coordinator, Internal Medicine including diabetes and pain, as well as Mom/Baby & NICU. This position will serve as a preceptor for students and residents.
In partnership with the Pharmacy Leader and other stakeholders, provide workflow management of the day-to-day operations in assigned pharmacy and/or clinical service area, driving patient care excellence while ensuring achievement of organizational goals. Is a key pharmacy liaison between service lines, programs, domains and clinical areas as assigned.
Foster an environment that includes consistent excellent patient and customer experience, outcomes that meet or exceed quality standards and financial targets, a culture of employee engagement and ongoing organizational and regulatory compliance.
Leads the day-to-day operations of assigned pharmacy and/or clinical department to ensure achievement of organizational vision, mission and goals.
Provide clinical, formulary and pharmacotherapy expertise and act as a resource to students, staff and interdisciplinary members of the care team. This may include coordinating student experiential rotation and acting as preceptor.
Provide clinical pharmacy services to all patients in assigned area using Standard Operating Procedures (SOP). This may include dispensing of medication, providing clinical services for patients, monitoring, adjusting patient therapies and documenting changes according to department policies and pharmacy regulations.
Act as resource for pharmaceutical care to patients, partnering with interdisciplinary care team members to ensure the appropriateness of medication order and therapy and the effectiveness, safety and convenience of the medication therapy.
Ensure commitment to agreed-upon patient care philosophies and standards by assessing the proper use of the practice model and creating or modifying workflow to support it.
Create and maintain an environment for pharmacy professionals to function efficiently with known accountabilities and exemplary performance standards.
Develop, implement and communicate key metrics and improvement activities to monitor the performance of pharmacy and/or clinical operations.
Partner with human resources and other teams on human capital initiatives that ensure recruitment of high quality candidates, foster staff engagement, encourage professional development and provide appropriate training and orientation.
Build powerful relationships with all internal groups, external agencies and other stakeholders to anticipate needs and ensure alignment within a highly matrixed environment.
Ensure contractual obligations are met.
Responsible for timely and accurate analysis and decision making to support pharmacy business performance, operational and financial objectives.
Drive a culture of continuous improvement using lean management tools to support, challenge and evolve processes to achieve system pharmacy goals.
Maintain knowledge, ensures and facilitates compliance with all relevant laws, regulations, policies, procedures and standards.
Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars and conferences.
Perform other responsibilities as needed/assigned.
Required Education: Doctor of Pharmacy or comparable experience
Required Experience: One year pharmacist experience in complex pharmacy operation or completion of an ASHP accredited PGY1 residency program
Required License/Registration: Minnesota Board of Pharmacist license and Registration as Preceptor. MUST meet ASHP preceptor requirements
Preferred Education: Completion of an ASHP-accredited resident Program
Preferred Experience: 3+ years of pharmacy experience in areas of speciality coverage
