To discover the career you want, for the future you deserve The Patient Aligned Care Team ( PACT) LPN serves the essential role of being the first direct care clinician to make contact with the veteran at his/ her appointment. The LPN checks the vital signs, weighs the patient, interview/complete patient clinical reminders, administer vaccinations and other injections, and communicates any relevant patient information to the medical provider. In addition, the LPN performs point-of-care ancillary lab testing for glucose, hemoglobin A1C, PT/INR, and 12-lead EKG when needed. The LPN interacts with patients and follow-up health promotion teaching, such as management of hypertension and diabetes. In addition, the LPN facilitates work flow to maintain efficiency in the clinic. The LPN must have the knowledge and skills necessary for self - direction in carrying out simple to most complex practical nursing care assignments for variable patient population requiring skilled nursing care. Work Schedule: Monday through Friday Between the hours of 7:30am - 5:00PM to include various shifts Telework: Not Available Virtual: This is not a virtual position.
