DiaNavi is a startup company developing a new paradigm for how digital products can help people with diabetes manage their care. We are looking to apply for and create both a DPP and DSME program. We are seeking candidates who are interested in leading the clinical development of our new programs. You will be in charge of creating these programs but we have a strong team of experts (data scientists, marketing experts, clinical professionals, software developers, etc.) willing to provide help and direct or indirect support.
Early on, this role will include creating and updating DPP and DSME content and applying for certification from the appropriate bodies. Once approved, the role will include clinical care for people with diabetes. Over time, as you grow the program, you will hire a team of Diabetes Care and Education Specialists and clinicians to support your work and the work of the organization. This is a dream job for a candidate who has seen firsthand the numerous challenges in the diabetes field and wants to utilize state-of-the-art data science, technology, and tools to improve the system. Diabetes care is too expensive, too infrequent, and not customized to the individual needs of the patient. These are all things we want to change. Our platform enables real-time feedback to a user that is based upon their unique behavior. Through deep analysis, our technology can identify subtle actions that may be causing spikes/drops in a user’s glucose. Our just-in-time messaging and coaching tools then provide users with a simplified distillation of the factors that are most influencing their glycemic response. Empowered with this information, our users can then engage in self-care behaviors.

Create, update, and maintain DPP and DSME content to ensure it is aligned with local, state, and national standards (e.g. ADA, ADCES, CDC).
Team-first attitude upwards, sideways, and downwards as a member of the leadership of the company
Desire to build out a high performing team by recruiting and retaining skilled Diabetes Care and Education Specialists, including mentorship of employees and contractors.
Lead DPP and DSME classes, including presentations, communicating with participants, and appropriate clinical follow-up.
A patient-first approach to care and a desire to provide exceptional clinical care
Provide patient-centered care aligned with recognized guidelines
Facile use of patient-centered language
Translate patient feedback into product improvements and new features.
Be the clinical champion and patient voice at the company
Coordinate with other internal or external providers supporting patients
Other duties as assigned
Advanced professional health-related certification (RN, PA, RDN, RPh, Pharm.D, etc.)
Experience working with patients in a diabetes related field
An interest in how technology can improve care for people with diabetes
A desire to utilize best practices and clinical guidelines to develop DPP and DSME programs, including owning Quality Improvement projects.
Ability to utilize Microsoft Office (Word, Excel, PowerPoint) or similar products (e.g. Google Docs, Gmail)
Familiarity with and interest in using phone apps, diabetes technology, and computers.
Travel (up to 10%)
