Educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition.
ESSENTIAL JOB DUTIES:
Provide outpatient diabetes and self-management training and education to patients who have Type 1, Type 2 or gestational diabetes.
Plan, develop, and implement individual and group diabetes education for adults, pediatrics, and their families and caregivers.
Instructs patients in the Standards of Care for diabetes.
Provide nutritional counseling as applicable for weight control and disease management.
Deliver patient education as needed or as directed by the Provider.
Develop collaborative relationships with other departments, services, and community health care agencies to facilitate and support quality diabetes care.
Attend outreach activities such as health fairs for diabetes screening and health education.
Evaluate the diabetes and health education program on a continuing basis; assesses teaching methods and education material for both staff and families of patients.
Monitor and evaluate short and long-term patient responses to therapeutic interventions in collaboration with providers; provides and maintains necessary follow-up for patients and families.
Performs related work as assigned.
Professional knowledge and skill as a certified diabetes educator
Ability to design, develop, deliver, and evaluate educational programs and materials
EDUCATION AND EXPERIENCE:
Bachelor’s degree in nursing or Master’s degree in dietetics
Certified Diabetes Care and Education Specialist
Preferred: 2-4 years working as a Certified Diabetic Educator
CERTIFICATE/LICENSE: Valid license based on location of practice as a Registered Nurse or Dietitian (must be registered by the Commission on Dietetic Registration)
