The Patient Aligned Care Team Clinical Pharmacy Specialist (PACT CPS) functions at the highest level of clinical practice and top of licensure and serves as an advanced practice provider working independently with prescribing privileges as outlined in the individual's practice area based scope of practice. The position is at CVAC Columbus, GA. The PACT CPS assists teams and patients with direct patient care by providing comprehensive medication management and disease management services including, but not limited to, diabetes, hypertension, hyperlipidemia, and other chronic disease management services. The PACT CPS with a scope of practice is integral to manage patients who have not reached or are not maintaining the intended therapy goals and those with medical conditions which are costly and associated with the use of multiple medications (e.g., cardiovascular disease, COPD, cancer chemotherapy, depression, and hypothyroidism) This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes (e.g., establishing and progressing towards individualized function-related goals, improving quality of life, minimizing theoretical risk of drug-related harm) through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas, and may perform other duties. The PACT CPS manages medication therapy by initiating, modifying and discontinuing medication independently. The PACT CPS follows patients in direct care setting to achieve therapeutic goal and make referrals as needed to optimize patient outcomes and treatment success. The PACT CPS refers to the appropriate level of care for patients when needed and is responsible for providing and documenting care of assigned patients. The PACT CPS has knowledge of VA/DOD clinical practice guidelines, The Joint Commission (TJC) standards, Veterans Affairs (VA) performance measures, VA Pharmacy Benefits Management (PBM) initiatives and will integrate these standards into practice. The PACT CPS may participate as a subject matter expert on PACT-related committees at the local facility level and as permitted at national and VISN levels. The CPS provider should assist with quality improvement projects related to PACT. Indirect patient care activities include teaching/mentoring trainees, participating in quality assurance and improvement projects, supporting research, staff education and consultation. The CPS provider will perform their duties within Central Alabama Healthcare System (CAVHCS) facilities and clinics or may work virtually using telehealth technologies when the team requirements do not necessitate a full time CPS provider be physically present. The CPS provider will be given appropriate resources and physical space (i.e. treatment room) for the direct care of their patients. Services provided by the staff are conducted via multiple modalities, including virtual and in-person care. Virtual modalities include Clinical Video Telehealth (CVT), VA Video Connect (VVC) and Telephone. This position may require travel to other CAVHCS facilities to include the Community Based Outreach Clinics (CBOC). Perform other duties as assigned.
