| UW Medical Center – Montlake has an outstanding opportunity for a full-time (100% FTE, 40 hours per week), day shift REGISTERED NURSE to join our Diabetes Institute in South Lake Union. The UW Medicine Diabetes Institute provides an interdisciplinary support system for patients with diabetes, including the realms of medicine, nutrition, and psycho-social interactions. Our providers include specialists in the treatment of diabetic nerve and kidney problems, evaluation and treatment of heart and vascular disease, the management of diabetes during and after surgery, and the use of intensive insulin therapy, including insulin infusion pumps. The UW Medicine Diabetes Institute also serves as an education and resource center for diabetic patients and their families, the community, and UW staff. The UW Medicine Diabetes Institute is a premiere clinic with many of the key opinion leaders in the field of diabetes, lipids, and diabetes research.
SCHEDULE:
The Registered Nurse will work 40 hours per week, Monday - Friday.
RESPONSIBILITIES:
The Registered Nurse will work closely with diabetes providers who are nationally respected in the field of diabetes management. The RN diabetes education team provides collaborative care through 1:1 appointments, and topic-specific group classes. The Registered Nurse will be responsible for triaging patient calls and eCare messages, following up on therapies and test results, managing prescriptions and referrals, and providing nursing advice in person, via phone and via telehealth.
REQUIREMENTS: - 2 years of current or recent medical-surgical nursing experience
LEGAL REQUIREMENTS:
- An active Registered Nurse license in the State of Washington
DESIRED QUALIFICATIONS:
- Current or recent nursing experience providing care services to diabetes or endocrinology patients
- Current or recent experience in an outpatient clinic
- Certified Diabetes Educator (CDE)
- Certified Diabetes Care and education specialist (CDCES)
- BSN or MSN
- Advanced knowledge of the Chronic Care Model (CCM) in relation to diabetes care
