THIS IS AN OPEN CONTINUOUS ANNOUNCEMENT. The incumbent will serve as a Physician in a Patient Aligned Care Team (PACT) setting. He/she will be responsible for providing medical care in an outpatient setting following PACT principles. May provide medical care using Telehealth for remote patients, or travel to other CBOCs to cover provider shortage. Duties include, but are not limited to: Utilizes all available tools, such as registries, to enable effective and efficient identification and intervention of individual patients and cohorts. Assures that clinical care provided meets all timelines and required documentation for the Chief of Staff (COS) office. Participates in multi-disciplinary team meetings to achieve a patient centered, holistic approach for clinical care and needs of the Veteran, focusing on continuous improvement of critical team processes. Ensures the patient's care plan contains medical recommendations for clinically indicated care. Ongoing, continuous care of one or more assigned panel(s) of Veterans providing health education and health coaching on wellness, disease prevention, chronic care management and self-management skills to patients and personal support persons. Collaborates with informatics technology staff to develop and implement systematized, electronically supported, standardized, tools to support PACT care delivery processes (e.g., pre-visit reminder calls, post hospitalization follow-up calls, recall scheduling procedures, new patient orientation, disease registries and primary care protocols for chronic disease management. Work Schedule: Monday - Friday; 8:00 am to 4:30 pm or as agreed upon with possibility of extended hours and weekends. NOTE: Candidates may be required to work weekends, irregular work hours and extended hours. The candidate must be willing to be assigned to other related services and/or locations, if necessary. ***Recruitment Incentive Approved: For Highly Qualified Candidates*** ***Relocation Incentive Approved: For Highly Qualified Candidates*** Student Loan Repayment/Education Debt Reduction Program (EDRP): This position may be eligible for the Education Debt Reduction Program (EDRP), a student loan reimbursement program. To participate in the program, you must meet specific individual eligibility requirements in accordance with VHA Policy 1021 and submit your application within four months of appointment. The final award amount (up to $200,000) and eligibility period (one to five years) is determined by the VHA Education Loan Repayment Services office. EDRP approval is contingent on this occupation and specialty being deemed "hard-to-recruit-and-retain" at the duty location you are interested in.
