Diabetes Educator, RN - Full Time, Days

Responsibilities Include:

• Providing clinical management and diabetes self-management education to diabetes patients in conjunction with LIPs. Planning and presenting programs to meet the assessed learning needs of healthcare providers, including patients and community, as required.
• Providing educational opportunities at times and places needed. Participating in developing, updating and maintaining educational materials. Enhancing diabetes education by involving patients, families and other caretakers as appropriate in the teaching and learning process.
• Formulating individualized plans of care based on assessment findings and interpreting the trends in specific patient populations to achieve best practice. Developing education programs incorporating aspects of cultural diversity of population served in a collaborative environment.
• Exercising discretion and judgment when providing nutritional assessments in collaboration with registered dietician and formulating individualized meal plans.
• Using an integrative, collaborative and multidisciplinary approach to diabetes care and education that results in enhanced patient outcomes. Demonstrating effective teaching skills using a variety of instructional methods and teaching strategies.
• Assisting with ensuring compliance with JCAHO, DOH, Magnet and other regulatory agency standards. Assisting in the maintenance of all required records for ADA certification and re-certification.
• Participating in activities that promote and expand the reputation and the services of the diabetes programs.
