About Our Job(Job Summary Description)
Provides professional nutritional & lifestyle coaching, diabetes education and clinical occupational health nursing care (for RN’s only) including assessment, preventive counseling, treatment and education to current Sarasota County employees and dependents 16 years of age and older. This position is primarily responsible for the administration of the Diabetes Education and Lifestyle Improvement (DELI) program and requires a Certified Diabetes Educator (CDE) designation; this position is also responsible for providing nutrition and lifestyle coaching while the occupational health nursing responsibilities are assigned as needed to RN candidates only. This position works on the Employee Health & Benefits team and requires team collaboration but is mostly autonomous. The position is managed by the manager of Occupational Health & Wellness and is under the direction of the Human Resources Director.
RD-CDE and RN-CDE:
Nutritional Coaching & Motivational Interviewing: Assess’ client’s dietary and lifestyle habits then advises clients using various motivational interviewing modalities to suggest dietary and lifestyle (exercise, stress, etc.) modifications/treatment plans for successful adaptation. Monitors and coaches clients toward obtaining their nutrition or health goals; mostly in weight-loss, hypertension, hyperlipidemia, diabetes and GERD diseases. Understands medical and pharmacy plan benefits and assists employees and dependents in efficient use of their benefit plans; refers to internal & community resources as needed.
Clinical Assessment: Reviews health/nutrition history and status, health risk factors, lab values (cholesterol, FBS, A1C) if available, drug/nutrient interactions, physical activity level, smoking & substance abuse.
Psychosocial/Environmental: Evaluates adequacy of food supply, household resources and food management skills. Assess’ client’s stress level, coping skills/strategies and beliefs related to food and nutrition and physical activity. Ensures chart documentation entries are made accurately and, in a HIPAA compliant manner. Conducts coaching sessions either in-person, telephonically, or via Zoom or MS Teams.
Certified Diabetes Educator (CDE): Provides comprehensive diabetes education by teaching employees and their dependents who are >16 years old and older (aka clients) about meal & nutrition planning, diabetes medications and injectables, exercise, blood glucose monitoring, insulin pump therapy (certification) and how to integrate diabetes care into everyday life. Assesses for potential practice gaps in care and client compliance; identifies learning needs and creates evidence-based programs to facilitate outcomes that provide quality care. Develops physician and vendor relationships, initiating and facilitating clinical communication with prescribing provider and pharmacy vendor for active client disease management and the administration of the Diabetes Education Lifestyle Improvement (DELI) program. Ensures chart documentation entries are made accurately and, in a HIPAA compliant manner. Conducts coaching sessions either in-person, telephonically, or via Zoom or MS Teams.
Research, Education & Marketing: Monitors nutritional trends and creates programs in response to analysis; actively solicits employee engagement via numerous resources (i.e. email, classes, videos, printed documents, 1:1 interaction, etc.) Conducts nutrition and/or cooking workshops and other varied disease management related classes. Creates video recordings of workshops for on-line learning. Develops professional presentations, articles and communications for both internal and external audiences. Performs program research and analysis, offering recommendations to expand the program offerings or improve client outcomes.
Occupational Health Nursing: Provides quality, competent evidence based occupational health nursing services such as; pre-placement, DOT and Public Safety employee physicals, periodic exams, respirator clearance (SCBA and N-95), EKG’s, hearing assessment, PPD’s, immunizations and other necessary testing as required by specific job descriptions of employees. Responsible for collecting specimens and routine laboratory tests such as, urine drug screens, breath alcohol, sputum samples, cholesterol and glucose (blood), vital signs, and other cultures/specimens as deemed necessary. Follows current universal precautions and infection control techniques. Ensures that COHC, BAT-DOT and other necessary occupational health credentialing is obtained after employment and maintains certification during course of employment. Occupational Health nurse training provided to qualified RN candidates.
Work Hours: Monday – Friday, 8:00 a.m. – 5:00 p.m.
Pay: $65,936.00 May Exceed Depending Upon Qualifications
Minimum Qualifications: A Bachelor’s Degree or higher from an accredited school of nutrition, food science or dietetics with a current, unencumbered & valid state of Florida Registered Dietician (RD) license. Four (4) years of professional dietician experience in a clinical setting; conducting nutritional assessments with subsequent nutritional counseling/coaching with clients whose medical needs range from gastro-intestinal issues, diabetes, hyperlipidemia, obesity, endocrine disorders, renal insufficiency and conditions of the liver. Must have the training and certification to be a Certified Diabetic Educator (CDE) with a minimum of two (2) years experience counseling diabetic clients on their nutritional, prescription and preventive care needs.
Graduate from an accredited school of nursing with a minimum of an Associate’s Degree or higher. Four (4) years of professional nursing experience. A current, unencumbered and valid state of Florida Registered Nurse (RN) license. Must also have the training and certification to be a Certified Diabetic Educator (CDE) with a minimum of two (2) years experience counseling diabetic clients on their nutritional, prescription and preventive care needs.
Job Competencies: Current Florida State Registered Dietician (RD) or Registered Nurse (RN) license. And hold a current Certified Diabetic Educator (CDE) certification.
