Your Role: Assesses nutrition status of patients with complex medical conditions. Develops, implements and evaluates outcomes of nutrition care plan for patients, ensuring appropriate use of oral, enteral, and parenteral regimens. Develops, implements and evaluates nutrition education and counseling for patients, families and caregivers, and the community. Participates in the training of dietetic interns, students, and other health care professionals. Participates in clinical nutrition outcomes research and conducts independent outcomes research in area of clinical practice. Provides clinical supervision, oversight, and mentoring of staff working in assigned practice area.
The Vanderbilt Eskind Diabetes Center is an internationally acclaimed facility that offers comprehensive care to both adults and children with diabetes. It offers a team approach to care that includes diabetes specialists, diabetes educators and nutritionists. For teenagers with diabetes, it also provides a seamless transition from pediatric to adult care.
The Vanderbilt Eskind Diabetes Center is one of only three diabetes centers in the U.S. accredited by the American Diabetes Association for treating adult, children's and gestational diabetes. We are one of the top five diabetes centers nationally for number of patients treated.

Ensure implementation of the National Standards for Diabetes Self-Management Education and Support and oversee Diabetes Self-Management Education and Support (DSMES) services in the Vanderbilt University Medical Center adult Eskind Diabetes Clinics in Medical Center East 8 and in Williamson County on Edward Curd Lane.
Ensure quality while being responsible for all components of DSMES including evidence-based practice, service design, evaluation, and continuous quality improvement.
o Maintain a three-ring binder(s) with required content and documentation for renewals and annual status reports
o Perform internal audits using the Review Criteria and Indicators to ensure that each of the ten National Standards are met
Arrange and coordinate Advisory Group activities including seeking input for DSMES services development, access, and/or improvement. Document enhancement of services reflecting external stakeholders input.
Liaise between the DSMES team members, the Advisory Group, other departments (e.g. Health IT, VMG Coding), and administration.
Monitor and facilitate maintenance of DSMES team members qualifications (CE credits, training, competence, licensures, and certifications).
Maintain ADA Recognition and participate in the evaluation of the DSMES service's effectiveness
o Collect and evaluate quality data to identify gaps in DSMES
o Implement the continuous quality improvement (CQI) process
o Communicate and provide feedback on the clinical outcomes and performance of DSMES services to team members, referring practitioners, and administration
Review ERP templates and resources to update elements as needed
Stay current on educational tools and resources for participants and educators
Complete and submit renewal applications every four years and status reports annually
Be prepared for and coordinate ADA and/or Centers for Medicare and Medicaid Services (CMS) audit(s) if randomly selected
Use ADA Audit Toolkit to ensure required documents from Reporting Period, Current Operations, and Required Documentation are in place
Ensure the ERP Quality Coordinator Guide is current per the Annual Requirement Tracker for a two person onsite audit 10 business days after notice
