Position Summary: The Diabetes Nurse is responsible for providing diabetes education for the Orange County Regional Diabetes Center at Fountain Valley Regional Hospital, including educational classes, diabetes screening assessments, and referrals associated with the clinic. The diabetes RN provides extensive data analysis and outcome reporting and works closely with the Diabetes Director. The Diabetes RN provides extensive education to staff and community regarding diabetes disease process, treatment, glucose monitoring and compliance. These services will include Diabetes education on specialty areas such as pediatrics and gestational diabetes. Demonstrates a positive image to the patients, families, and public Develops and implements services to meet patient and physician expectations. Demonstrates excellence and professionalism in the practice through actions that are consistent with established professional practice guidelines and established local, state and federal regulations. Develops a plan for professional development and sets goals for further development. Works in partnership with the patient his or her family, significant others, and other health care providers to define outcomes and processes to achieve them. Shares the patient individualized plan and progress with referring providers.
Essential Duties 1.Performs staff duties as needed. 2.15 hours of Continuing education are required in Diabetes Education and Heart Failure annually. 3. Actively participates in the hospital Performance Improvement (PI) Program. 4.Consistently apply infection control policies/practices. 5.Meet population specific competencies. 6.Attend department specific education/training, in-services, and staff meetings. 7.Collects, prepares, and plans educational programs through personal interviews, group meetings, and library research; conducts preliminary analysis of such data; develops program services. 8.Participates in teaching community classes and screenings when appropriate at designated local community locations; ensures materials, equipment, and facilities are arranged and developed accordingly. 9.Responsible for all data collection and reporting requirements established by the Orange County Regional Diabetes Center. 10.Performs special projects or other related work as requested
Licenses and Certifications: Registered Nurse from accredited nursing program Certified Diabetes RN American Heart Association BLS Certification BSN with Public Health Nurse Certification Preferred Certified Pump Trainer/Heart Failure Nurse Certification Preferred
Education and Experience: Bachelorï¿½s Degree in Nursing preferred. At least two years experience in a Supervisory capacity in the area Public Health, preferred.
