Certified Diabetes Educator Overview:
A CDE provides clinical expertise in diabetes management, in collaboration with the team of clinic providers. This role also provides education and treatment plans to diabetic patients and successfully help them reach their disease management goals. In addition, the CDE will create and develop programs within the 5 clinic locations to achieve positive change in the diabetic patient population.
This is a full-time position and requires travel to multiple clinics in the Charlotte area.
A Certified Diabetes Educator at OurHealth will do the following on a regular basis:
Follow guidelines of the American Diabetes Association (ADA) and work with newly diagnosed diabetic patients on a comprehensive education plan encompassing nutrition, exercise and blood glucose monitoring Develop treatment plan and discuss proposed course of treatment with patient’s provider Collaborate regularly with providers, as well as other members of the team, including health coaches Work with poorly controlled diabetic patients or patients who have not reached their disease management goals, focusing on areas where improvement in control of disease could be accomplished Promote self-management to achieve individualized behavioral and treatment goals that optimize health outcomes Organize group programming for diabetes patients at various clinics and work sites, as deemed necessary Attend outreach activities such as health fairs for diabetes screening and health education Instruct patients and family members on how to administer insulin, self-monitor blood glucose, and medication education Assist in the development of policies and procedures for diabetes and health education guidelines
A successful Certified Diabetes Educator at OurHealth will have the following:
Certified Diabetes Educator credential required Current registered nurse license preferred Health coaching certification from an accredited health coaching program, such as WellCoaches preferred Minimum of two years’ direct patient care with diabetic population Experience working with electronic medical record systems and other technical tools Understand medical terminology Knowledge of computers & electronic medical record systems. Effective time-management skills and ability to successfully manage and complete a variety of tasks A commitment to OurHealth’s mission Knowledge of OurHealth’s policies and procedures Team-oriented attitude Self-awareness to ask questions when help is needed Solid communication skills: written, verbal, and listening Ability to motivate and develop team members. Ability to maintain confidentiality Willingness and ability to travel to different OurHealth clinics
