Johns Hopkins All Children's Hospital is seeking a Clinical Diabetes Educator, Dietitian to join our team. The dietitian provides individualized care for all aspects of medical nutrition therapy (MNT) and diabetes self-management including meal planning, diabetes medications, exercise, how to check blood glucose and how to integrate diabetes care into everyday life. S/he works collaboratively with a multi-disciplinary health team to promote wellness, maintain current health, and/or intervene in acute or chronic illness in the pediatric population.
Educates patients and families on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing.
Plans, coordinates, implements and evaluates nutritional interventions based on the assessment of the patient/family to achieve progress toward desired clinical outcomes.
Documents expected outcomes or measurable goals, interventions performed and response to assist clinical practitioners including physicians in care coordination.
Collects patient data to monitor health status and proactively manage the patient’s condition.
Collaborates with other health care professionals to provide diabetes self-management education (DSME) and diabetes self-management support (DSMS) to individuals and groups.
Facilitates learning using education to close knowledge gaps.
Develops data driven educational programs and learning opportunities. Utilizes evaluation methods to measure and report outcomes.
Bachelor’s degree in a related field from an accredited college or university; master’s degree preferred.
Registered Dietitian/Nutritionist by the Commission on Dietetic Registration, Academy of Nutrition and Dietetics
Dietetics/Nutritionist Licensure in the State of Florida
