Here is an excellent role for a skilled professional nurse with competence in diabetes and lifestyle medicine who will thrive within a multidisciplinary framework and actively promote holistic care for patients and families. The self-directed professional we select will demonstrate excellent problem-solving ability and initiative in a role that encompasses the duties of nurse clinician, educator, and consultant. Additional responsibilities will include:
Providing direct patient care and/or consultation using in-depth clinical knowledge and skills in the area of diabetes.
Serving as an advocate and role model for "Patients First" in the best interests of patients and their significant others, coworkers, the community and the nursing profession.
Mentoring staff to ensure quality of healthcare practices.
Participating in the assessment, problem-solving, decision-making, and evaluation of clinical/system issues.
Championing collaborative practice with coworkers and other disciplines and departments.
Participating in programs to meet continuing education requirements.
Providing formal diabetes-related education to internal and external groups.
Developing and presenting educational materials in support of specific behavioral goals.
Facilitating research-based practice in the clinical area of diabetes/lifestyle medicine.
Collecting and documenting quality management and outcomes research data.
Serving as a resource for community services available to patients with diabetes.
Assuming responsibility for meeting regulatory and other requirements.
Current licensure as a professional nurse in the state of Illinois.
Current certification in Basic Life Support (BLS) at the healthcare provider level.
Certification as a diabetes educator.
Bachelor's degree from an accredited nursing program.
Demonstrated exceptional proficiency in the areas of diabetes self-management training, including but not limited to self-monitoring of blood glucose, risk reduction, medications, acute complications and the diabetes disease process.
Extensive knowledge of diabetes program management.
Strong problem-solving skills and the ability to interact/collaborate effectively with others during difficult situations.
Excellent communications skills essential to effective teaching and leading.
Two or more years of experience in diabetes care.
Master's degree from an accredited nursing program or in a related field.
