Tallahassee Memorial HealthCare (TMH) is searching for a dynamic, full-time, Registered Nurse Patient Educator to join the interdisciplinary team performing patient care in our Diabetes Education and Management services at the NEW Metabolic Health Center.

Tallahassee Memorial HealthCare is looking for a registered nurse educator who is passionate about coming alongside & educating our patients and families impacted by this life-threatening disease.
The Diabetes Education and Management services are housed within the new outpatient Metabolic Health Center. These services focus on patient education and disease management with the diabetes population served by the Metabolic Health Center, TMH, and for referred patients from the community. This position works in a team-based environment with many diabetes educators for support and to help extend diabetes education and management services and outreach to our community.
Utilizes the nursing process and adult-child learning theory to assess, plan, provide, and evaluate direct patient teaching/counseling to persons diagnosed with diabetes mellitus and related health conditions. Coordinates and case manage the array of services to patients as needed. Opportunities to become certified in diabetes education, insulin pump training, and continuous glucose monitoring through growth and development in this position.
•Acts as liaison and primary point of communication with referring/managing physician, education team, and Diabetes Center medical directors to identify diabetes education and management goals, and clinical alternatives to improve patient status. •Provides services in individual, group, off-site and other designated locations under the supervision of Director and Assistant Nurse Manager. •Plans and participates in professional training and outreach/marketing activities as initiated and directed
•Registered Nurse (Active Florida RN License or in process of obtaining active FL RN Licensure will be considered) •Bachelor’s of Science in Nursing (BSN) •2 or more years of experience preferred •Strong patient focus and commitment to continuous process improvement •Ability to proactively inspire & put the patient first •Commitment to life-long learning •CDE not required but will be a goal for the position •Previous clinical and/or educational experience with patients with chronic conditions preferred
