The position is accountable for the coordination and execution of an appropriate and timely discharge plan for a designated caseload of patients. The discharge planner, under the direction of the Care Manager and order of the physician, collaborates with the patient/family and members of the health care team, to ensure a safe and effective transition through the continuum of care.
Active LVN Licensure in California and / or Bachelors Degree in Social Work or related Social Sciences.
Minimum of 1-2 years experience in Discharge Planning in an Acute Hospital or Skilled Nursing Facility.
Candidate must be able to multitask and prioritize efficiently.
Excellent verbal and written communications along with great customer service skills are essential.
Weekend per diem position with coverage availability for weekdays.
Bachelor’s degree in Social Work or related Health Sciences, or CA licensure as an LVN preferred.
