The professional practice of nursing at JHACH involves the promotion of a culture of innovation and a commitment to academic growth and professional development. Contributes to the professional development and growth of nursing personnel in the practice environment. Operationalizes services by ensuring objectives and goals align with the division and organization strategic goals. Assesses ongoing diabetes education for all staff involved in the care of patients with glycemic issues, glycemic control monitoring and protocols. Participates in the development of other nursing professionals through consultation and evidence based education. Responsible for collaborating with other health care professionals to provide diabetes self-management education (DSME) and diabetes self-management support (DSMS) to individuals and groups. Evaluates documents and communicates patient response to interventions and revises plan of care to achieve self-care. Maintains active participation in his/her own continued education and professional development. Partners for academic growth. Develops and provides family and patient education including community programs.
1. Facilitates learning using education to close knowledge and practice gaps.
2. Develops data driven educational programs and learning opportunities, and uses evaluation methods to measure and report outcomes.

•Bachelors of Science in Nursing (BSN) required for external hires within 36 months of hire date.
•Diabetic Educator Certification required.
•3 years pediatric clinical experience and evidence of participation in educational activities.
•Knowledge and application of adult education learning theory and educational techniques.
•Experience in designing and implementing educational programs.
•Experience with evaluating educational outcomes.
