Position Summary: The Diabetes Educator is a healthcare professional with the knowledge, skill, and expertise for the comprehensive management of diabetes. Through coordination with the provider, provides comprehensive diabetes related care to patient with diabetes and/or associated conditions, such as diabetic complication.
Responsibilities: 1.Assess and monitor glycemic control in the patient with diabetes. 2.At the patient’s initial appointment, review medical record and obtain a complete history as it relates to chronic disease process, patient’s knowledge about diabetes, and technical skills. 3.Provide comprehensive management of diabetes and associated conditions, including adjusting patient’s current medication and adding additional oral agents or insulin injections to patient’s regimen, per approved protocols. 4.Provide comprehensive management of chronic disease and assess condition including medication management per protocol. 5.Review and monitor medications and treatment plans. 6.Provide, develop, and evaluate patient education material. 7.Act as primary contact for patients and families for issues related to patient’s chronic disease care. 8.Develop a plan of care that will optimize the patient’s control of chronic disease, to screen for complications, and eliminate knowledge deficits related to chronic disease and communicate with primary provider. 9.Order and obtain lab work to assess patient’s control of their chronic disease, to screen for complications and identify risk factors following the American Diabetes Association standards of care. 10.Facilitate referral to other community healthcare facilities, agencies, providers and/or resources. 11.Communicate plan of care with patient’s primary provider. 12.Document patient status in medical record as it relates to chronic disease management. 13.Provide ongoing follow-up for management of chronic disease.
Teaching/Consultation: 1.Provide consultation to SSM Health Dean Medical Group health care providers related to chronic disease and associated conditions. 2.Provide continuing education opportunities for clinic staff and serves as a resource. 3.Serve as part of the instructional team for the Diabetes Self-Management Education (DSME) classes. 4.Participates collaboratively in the implementation of the DSME program with the Program Coordinator and other Department Personnel. 5.Assists in the development and implementation of the DSME curriculum.
Qualifications: Required: 1.State licensure as a Registered Nurse or Registered Dietitian with certification from the American Dietetic Association. 2.Certification as a Diabetes Educator (CDE) or CDE eligible, within one year of hire, with required Continuing Education Units. Preferred: 1.Master’s Degree. 2.Outpatient clinical experience in the area of diabetes management and/or other chronic disease management.
