This position performs a complete assessment of the patient addressing medical, psychological, cultural, ethnic and religious / spiritual needs. Assesses diabetic education needs. Reassesses as needed. Establishes goals and develops an educational plan for client. Implements education plan for the client and family members / significant others in regards to the knowledge and skills appropriate for “self care” related to diabetes. Evaluates effectiveness of client / family learning / teaching process and modifies plan as needed. Works cooperatively with the other members of the healthcare team in order to assure a quality learning experience for the client and family / significant other. Documents patient assessment both admitting and discharge, the plan of care, education, modifications of plan, progress notes, educational checklist, etc. in the medical record in a timely manner according to the state practice act and hospital policy. Demonstrates the knowledge of the principle of growth and development over the life span on the client and the skills necessary to provide age appropriate care for the client served. Assists in the evaluation of the Diabetes Program. Assists in the setting up and teaching of Diabetes Self-Management Course.
Education: Minimum of an Associate Degree in Nursing is required.
Experience: Experience in the care of persons with chronic disease required.
Licenses: Current RN license from the State of NE required.
