Job Summary: The Nurse Practitioner Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manages the patient's condition.
Essential Responsibilities: Develops and implements a plan of care, including treatments, medications and the like within defined guidelines for practice. Provides care to the patient in a manner that incorporates the principles of patient satisfaction and delivery of cost effective care. Participates in the development and evaluation of practice guidelines. Provides diabetes education for patients in the practice. Serves as the liaison among the practice, providers, patients and practice leadership. Provides overall coordination for diabetes education, in conjunction with the providers in the practice, including supervision of all education materials. Monitors the individual's response to therapy and revises the plan, as indicated, with particular attention to education and counseling and encouragement of self-care. Provides ongoing individualized patient orientation to BIDHC/BIDCO. Seeks consultation from the consulting physician and other members of the healthcare team. Makes appropriate referrals as indicated by patient needs. Develops education content and policies and procedures. Provides assessment, teaching, and follow-up instruction to Diabetes Education participants.Coordinates the planning and review of the diabetes education program each year. Working with Practice Manager, physicians, and Sr. Leadership, evaluates diabetes education patient effectiveness. Makes adjustments to the program annually or as needed. Answers incoming calls from doctors and patients. Triages calls from patients and other physician offices. Facilitates urgent and routine patient appointments as needed. Answers patient questions as appropriate.
Required Qualifications: Master's degree in Nursing required. License Nurse Practitioner License required., and Registration Basic Life Support required. 3-5 years related work experience required. Knowledge of the recommended American Diabetes Association¿s Standards of Care for people with diabetes. Certified Diabetes Educator (CDE) Advanced skills with Microsoft applications which may include Outlook, Word, Excel, PowerPoint or Access and other web-based applications. May produce complex documents, perform analysis and maintain databases. Preferred Qualifications: Certification in Diabetes Education. Completes a minimal of 12 hours or more of Continuing Education in Diabetes Education principles and behavioral strategies every two years. Member of the American Association of Diabetes Educators.
