Wood County Hospital is now hiring an outpatient Dietitian. The Outpatient Dietitian provides personal care and professional excellence by coordinating, planning and providing Medical Nutrition Therapy and Diabetes Self-Management Training for patients. Dietitian will work with community, physicians and staff on nutritional and diabetes education programs. This individual will be working with patients in both the outpatient setting and our CPC+ physician practices.
Job Duties: Assess and provide Medical Nutrition therapy for outpatient clients, Develop plan of care for Diabetes Education clients, collaborating with other members of the Diabetes Education team, Assess Diabetes Education clients with Pre-Diabetes, Type 1, Type 2 and/or gestational diabetes, Instruct diabetes education clients through classes and/or individual sessions, Monitor Gestational Diabetes clients along with physicians, weekly assessment of diet and blood sugars
Hours: Full Time, 40 hours/week
