NO Holidays!* 26 Days Paid Time Off! 10 Paid Federal Holidays! * Comp Time for Occasional Evening or Weekend Hours Worked!* Annual CME Stipend!* Malpractice Insurance Provided!DUTIES: The duties include but are not limited to the following: * Manage care for the patients of the primary care practice within the Population Health domain to promote effective education, self-management support, and timely health care delivery to help improve patient’s health and wellness efforts. * Collect, analyze, interpret and evaluate data to assess trends and developments to target the population and deliver Health Promotion and Wellness (HPW) services directly to patients.* Be part of a multidisciplinary team that assesses health promotions risk, analyzes health data, develops content for educational programs, drafts health promotions policy guidance, and evaluates health promotions outcome measures aim to deliver improved patient care. As requested, the HCW will provide input and technical guidance on issues related to the Health Promotion and Wellness (HPW) program. * Coordinate and deliver reports to BUMED Population Health teams. Collaborate and communicate between BUMED Populations Health and Naval Medical Center San Diego Medical Home Port teams. * Work with all clinical teams as a resource on care management of all patients of the practice. Improving patient care, quality of life and compliance are the focus to include the following: 1) Pre-visit planning to ensure information and care completion prior to visit when possible. 2) After visit summary review with patients whenever appropriate. 3) Provide follow-up contact with patient to ensure compliance with recommendations. 4) Educating and involving patients to improve their health-patient engagement. 5) Assisting patients to set goals, progress and identify barriers when patient not meeting treatment goals. 6) Serve as the resource across the continuum (consulting physicians, hospitals, ER and other frequently used healthcare resources) and the patient. Manage many aspects of patient’s care: referrals, hospitalizations, ER visits, ancillary testing and other as directed to achieve optimum outcomes in safe and cost effective manner. 7) Develop a list of medical supply and community resources available to patients.* Be required to successfully complete the Navy and Marine Corps Public Health (NMCPH) Center Health Promotion training to include Health Promotion and Wellness Course and Patient Centered Prevention Counseling. By the end of this course, the HCW must verbalize understanding of Department of the Navy (DON) Health Promotion and Wellness campaign based on National Prevention Strategy as well as the 21st Century Sailor and Marine initiative.SCHEDULING & STAFFING:* Work Schedule: The RN Certified Diabetes Educator may be scheduled to provide services between the hours of 0600 and 2100 Monday through Friday and 0700 and 1300 on Saturday or Sunday. Each shift is to include an uncompensated 30 minute lunch break. A typical schedule would be 0730 – 1630. The RN Certified Diabetes Educator shall be on duty for about 40 hours each week, except during periods of approved leave and holidays. In no instance will the RN Certified Diabetes Educator be required to provide services (on-site service plus approved leave and holidays) in excess of 80 hours per 2-week period. The specific schedule for each 2-week period will be scheduled 1 month in advance by the supervisor.NOTE: In addition to the normally scheduled work hours given above, the RN Certified Diabetes Educator will occasionally be required to teach evening classes and periodically participate/present in a health fair on the weekend. Compensatory time off will be granted for evening or weekend work on an hour for hour basis. Earned compensatory time shall be used within two pay periods.* Duty Location: The RN Certified Diabetes Educator shall provide services in support of Naval Medical Center San Diego. The primary site of services shall be located in the Primary Care Clinic in support of Medical Home Port. Services may also be required in support of the medical home ports at NMC San Diego, and associated branch health clinics. Locations Include: Naval Branch Health Clinic Naval Training Center, San Diego, CA; Naval Branch Health Clinic Chula Vista/Eastlake, CA; Naval Branch Health Clinic Kearny Mesa, CA; Naval Branch Health Clinic North Island, Naval Branch Health Clinic Naval Station 32nd Street, Naval Branch Health Clinic Rancho Bernardo and Naval Branch Health Clinic Miramar.NOTE: Travel reimbursement will be provided when traveling to associated branch health clinics.
QUALIFICATIONS:* Education: Possess a Bachelor's OR Master’s Degree in Nursing, Health Promotion, Education, Community/Public Health or Wellness, or a Health Science related field from an accredited university.* Licensure / Certification: 1) Maintain a current and unrestricted license to practice nursing in any state. 2) Possess certification as a certified diabetes educator (CDE). 3) Maintain a current BLS certification (American Heart Association – Healthcare Provider).* Experience: Possess at least two years of full-time experience as a health educator within the last four years working with Health Promotions and Wellness campaigns based on the U.S. Surgeon General's National Prevention Strategy.*Must be a U.S citizen and have the ability to pass a background check/security clearance.
