Overview Pediatric Specialists of Virginia (PSV) is seeking a Certified Diabetes Educator to assess and facilitate the needs of patients with diabetes. The Certified Diabetes Educator will be responsible for designing, implementing, and evaluating individualized teaching plans for these patients.
Knowledgeable in all aspects of diabetes care, and acts as resource for patients, community, and hospital personnel.
Integrates knowledge, skills, and experience to meet the multifaceted needs of diabetes patients and families throughout the continuum of care.
Assesses individualized educational, behavioral, and clinical needs, facilitates goal planning, provides patient centered education and evaluates desired outcomes.
Collects and interprets complex data, recognizes subtle changes in the patient/family condition. Demonstrates through documentation.
Provides individualized care in all aspects of diabetes self-management to patients and their families, as demonstrated through patient feedback and satisfaction surveys, referring provider communication, and documentation.
Utilizes interactive teaching/learning strategies that are age appropriate and adapted to group and individual interventions. Demonstrated via observation and thorough documentation of achievement of desirable patient behavioral outcomes.
Provides clinical and educational interventions with consideration of the cultural, ethical and spiritual values and needs of patients & their families. Demonstrates by use of culturally appropriate materials and use of certified interpreters as needed.
Collaborates on clinical decisions with multidisciplinary team; seeks validation from mentors for complex patients or unfamiliar situations. Demonstrated by professional observation and positive patient outcomes.
Plans, develops and implements new programs to meet the needs of the patient population and enhance diabetes self-management education within PSV. Participates in activities that promote and expand the reputation and the services of PSV.
Participates in community and professional outreach that increases overall awareness of diabetes. Demonstrates through participating in a community or professional program annually.
Participates in self-directed work teams to evaluate existing programs, identify need for new programs, and assist in development or revision as appropriate. Demonstrates by completing work assignments by due date.
Takes proactive measures for updating patient educational materials. Initiates updates based on national standards of DSME/T or new clinical research data.
Communicates appropriate information with other team members and documents all patient care interventions.
Utilizes standards, and evidenced based practice for delivery of care and education.
Incorporates new data and learning to continuously enhance patient outcomes. Applies quality improvement methodology to improve overall diabetes services.
Reviews/revises standards and guidelines to be in line with current national standards.
Ensures improvements in diabetes practice by assuming responsibility for self development in life-long learning.
Minimum Qualifications Required
One (1) year or Five Hundred (500) hours of direct diabetes education within the last two (2) years; or two (2) years experience in clinical care of patients with diabetes
Minimum two (2) years experience as Registered Nurse
Fifteen (15) CE units per year in DSME/T
Eligible to sit for the CDE within two and one-half (2½) years of employment
Current AHA BLS certification
Current RN Licensure
Current Membership in one (1) of the following: American Assoc Diabetes Education, American Diabetes Association, or American Nurse Association
All employees must pass an employment TB, drug and physical exam
