In this position, you’ll help coordinate and implement health education, prevention and promotion services for patients diagnosed with diabetes. Perform thorough assessments to determine their educational needs. Identify their specific diabetes self-management and medical nutrition therapy goals, and develop individualized educational care plans based on your findings. Conduct group training and behavioral interventions. Contribute to continuous care quality improvement, and feel the pride of a better tomorrow.
Current NYS RN license
Degree from an accredited nursing program
