CHOP is currently hiring a Diabetes Educator, in Philadelphia, PA, to provide diabetes self-management education and patient support for patients with Type 1 and Type 2 Diabetes. In this role, you will teach families in both classroom and one-on-one education settings, develop and implement patient education and treatment plans, and maintain systems to support American Diabetes Association recognition as a center of excellence. You will develop resources for clinicians including technology updates and practice guidelines to support their use, as well as collect, track and analyze data to evaluate and improve the diabetes educational systems and patient outcomes.
To qualify for this role, candidates must have a PA State RN license or Dietitian-Nutritionist license and be certified as a Diabetes Educator through the National Certification Board for Diabetes Educators or be CDE eligible with expectation of successful credentialing within 90 days of hire. A BS in Nursing or BS in Dietetics is required. A minimum of one year of pediatric diabetes-related experience and experience managing patients on insulin pumps and CGMS systems is highly preferred.
This role will be responsible for assisting families in making decisions about their diabetes program including: • Dietary and carbohydrate intake and use of insulin-to-carb ratios and correction factors • Insulin dose adjustments for MDI, Basal/Bolus programs and insulin pump therapy • Exercise and activity planning using carbohydrate/insulin adjustments
An ideal candidate would have expertise in downloading and interpretation of blood glucose meter, pump and CGM data using existing practice guidelines.
