In collaboration with the Director and the Palomar Health Nursing leadership, responsible for implementation of evidenced based practice and diabetes care standards identified by professional organizations along with Federal and State regulations. Consults with the patient and family to 1) Identify educational learning needs, 2) support medication management in the hospital; 3) assist in developing and providing safe transitions of care and discharge. Serve as a mentor to nursing staff, students and other disciplines. Committed to demonstrating improved outcomes, assist in the development of interdisciplinary educational programs to enhance nursing care and clinical knowledge. Implement programs that contribute to patient safety and by identifying opportunities to promote optimal metabolic management. Work collaboratively with unit CNS/Educator. Emphasis is placed on the needs of the patient, to build strong relationships with the medical staff, nursing staff and ancillary departments to best meet patient needs, and fulfill the Palomar Health vision. Promote continuity and quality of care through the use of data and the integration of the performance improvement process. Collects and analyzes data to identify trends, opportunities for improvement and establishes associated plan using the PDCA Model to improve patient outcomes under the direction of the Diabetes Nurse Specialist. Participate in system-wide performance improvement projects. Speak and read English at a level that is sufficient to satisfactorily perform the essential functions of the position. Knowledge of standard office equipment (i.e., calculator, fax, photocopier) and personal computer and computer software skills (i.e., MS Windows, Excel, Access, Word, Powerpoint, internet, e-mail). Windows computer skills including proficient use of keyboarding, use of mouse or keys for functions such as selecting items, use of drop down menus, scroll bars, opening folders, copying and similar operations required upon employment or within the 1st two weeks of employment to perform the essential functions of the job.
