RN/RD/CDE-Senior Hospital Diabetes Clinician Full-Time, Days Bon Secours Virginia Health System (St. Mary's Hospital in Richmond, VA/Memorial Regional in Mechanicsburg, VA)
Collaborating with the program director, physicians, Resource/Utilization/Case Management, diabetes staff and other hospital personnel, the confident practitioner we select for this important role will assist in the achievement of program objectives and help to enhance clinical outcomes and customer satisfaction. This seasoned, experienced professional will assess each patient's understanding of the disease process—including chronic complications, standards of care, and current areas requiring clinical intervention—via interview and self-assessment and/or medical record. Additional responsibilities include:
Assisting patients to set self-care goals to improve disease management.
Providing education and implementing interventions: medical nutrition therapy, self-blood glucose monitoring, medication/injectables, and insulin-pump therapy (if certified).
Exploring strategies to maintain and/or reduce resource consumption and length of stay.
Reviewing case variances and outcomes to improve organizational and clinical systems of care.
Facilitating case management of complex diabetes cases through diabetes inpatient clinicians, case managers, social workers, and other clinicians.
Functioning as case manager, educator, dietitian, or nursing coordinator as needed.
Advising patients on diabetes wound care.
Current, active RN licensure as a registered nurse or registered dietitian in the state of Virginia.
5+ years of related experience within a hospital setting.
A full understanding of diabetes and the ability to resolve a wide range of issues in creative ways.
The good judgment to select optimal methods and techniques for obtaining solutions.
Ability and willingness to travel approximately 10percent of the time.
Certified diabetes educator through the NCBDE.
