Hours: Full Time; Monday - Friday; Variable Shifts Location: The Polyclinic Madison Center
Provide patient-centric, self-management based diabetes education to individuals and targeted populations integrating the Chronic Care Model and Six Institute of Medicine AIMS. Collaborate with and function as an integral part of an interdisciplinary Cardiometabolic Risk Reduction team.
Requires a Bachelor's Degree in Nursing, an RN License in the State of Washington; and CDE (Certified Diabetes Educator) preferred and/or commitment to attain within 18 months of hire.
In addition, requires a minimum of three years of clinical nursing experience in inpatient and outpatient settings; knowledge of diabetes and pathophysiological changes with the disease process; working knowledge of care of patients with gestational diabetes desired; and experience with Continuous Glucose Monitoring, insulin pump starts and monitoring, and insulin administration and pens (to include other pens, i.e., Byetta) preferred.
Must have proven work experience in program/project management, implementation, and evaluation; demonstrated expertise in curriculum development, classroom teaching and production of education materials; and knowledge of JCAHO and ADA standards as well as state requirements for care and treatment of children and adults with diabetes. Working knowledge of quality assurance functions and NCQA guidelines preferred.
