We are looking for Certified Diabetes Educators to pilot a new holistic diabetes management program designed for people living with Type 2 Diabetes. As we develop this program, our mission is to support and accelerate people’s personal journey to a happy and healthy life beyond managing diabetes. Working side-by-side with a patient, this role will incorporate the patient’s mind, body, and environment to offer practical and actionable steps to better understand and control their health and wellbeing.
The purpose of this role is to be the compassionate, human support for our participants in the program — providing guidance, encouragement, and collaboration to figure out what works for them and the realities of their life.
We’re looking for CDEs who have the expertise, but want to practice a different, more individualized approach to supporting people with Type 2 Diabetes. While you are working with patients in this pilot phase, you will also be helping our team design and develop the role and necessary tools to do the job well. This is an opportunity to craft a new approach for diabetes support that will be scaled to a larger, global patient population over the next few years.
Collaborate with the IDEO design team to design and prototype this new kind of support role with Type 2 patients as we develop and refine this integrated diabetes self-management program.
During the 3 week and/or 3 month pilot, the Diabetes Educator will be assigned a set of participating patients that they will support across the span of the pilot.
Daily responsibilities during the pilot:
developing rapport with individual participants
identifying challenging areas and needs for the patient in managing their diabetes
recommending short, time-based challenges for the patient to try to get in better control of their diabetes, using a provided set of digital and physical tools / resources.
Must be a Registered Nurse or Registered Dietitian with Certified Diabetes Educator accreditation and minimum of 5 years experience. Bachelor’s degree in health promotion, health education, nursing, athletic training, nutrition, counseling or related field is preferred.
Share excitement for our patient support approach — providing personalized and individual support to a patient between office visits, through everyday habits.
Must be open and interested to try something different than traditional diabetes education. Must be open to a collaborative and iterative process in order to explore and prototype the design of this role. We want you to design with us!
Minimum of 5 years of experience in supporting Diabetes Type 2 patients. Prefer outpatient experience.
Comfortable communicating with patients over phone and chat. Excellent verbal and written communication skills.
Must be comfortable using online tools and logging into a portal for all patient interactions.
Must have fluency reviewing and interpreting patient’s data and trends in order to provide personalized support.
Bi-lingual (Spanish/English) highly desirable.
