The Polyclinic - Seattle, WA Hours: Part Time (38 hours per week) Location: The Polyclinic Madison Center & The Polyclinic Northgate Plaza
Develop, implement and provide clinical nutritional assessment, monitoring, care planning, education and counseling services to patients in an ambulatory, office setting. Perform duties in support of and in compliance with performance improvement plans, and other licensing, accrediting and regulatory agencies. Provide diabetes education to individual patients, as well as to groups in a class setting. Assess and identify the individual's specific education needs. Provide training on insulin pump therapy and continuous glucose monitoring as well as instructing patients on proper use of insulin administration.
Requires a BA in food and nutrition, dietetics, nutritional sciences, human nutrition or comparable major; a minimum of three years of nutrition experience in inpatient and outpatient settings; proven work experience in program/project development, implementation and evaluation; and demonstrated expertise in curriculum development and production of education materials. Must be a registered dietician (RD) by the Commission on Dietetic Registration.
Prefer a Master’s Degree in related field, and CDE (Certified Diabetes Educator) and/or commitment to attain within 18 months of hire. Experience as a certified insulin pump trainer with specific experience with Medtronic, Omnipod, and Tandem a plus.
