Derry Medical Center, a physician-owned, independent practice, is seeking a personable, motivated, and experienced Certified Diabetes Educator to join our growing team of primary care providers.
This full-time position will work closely and collaboratively with nursing staff and providers to provide diabetes education to patients and families.
** This position is open for a nurse or a registered dietitian who is a certified Diabetes Educator. **
•Provides Diabetes Self-Management Education (DSME) training to patients with Type 1 and Type 2 diabetes
•Discusses signs, symptoms and treatment of Hyperglycemia, hypoglycemia, sick day management, medication instruction, insulin pump management education, continuous glucose monitoring education, and nutrition assessment
•Plans and implements diabetes education for adults and their families/caregivers
•Participates and organizes diabetes outreach activities to promote diabetes prevention and management for the community
•Trains and educates nursing staff in diabetes survival skills; coordinates in-service training, continuing education, and other related diabetes and health clinical training
•Instructs patients and family members on how to administer insulin, self-monitor blood glucose, and medication education
•Monday thru Friday (day & evenings)
•No weekend hours

•Bachelor of Science in Nutrition or Bachelor of Science in Nursing
•Minimum of two (2) years of experience in clinical dietetics or nursing and as a certified diabetes educator
•Registered Dietitian with NH license or RN with NH license
•Current Certification as a Diabetes Educator by NCBDE required
•Certified pump trainer preferred
•Experience in Electronic Medical Records
