Confluence Health is seeking a Diabetes Specialist to perform Diabetes Self-Management Education and Training for inpatient and TCU/Rehab patients through assessment, evaluation, education, medication management, planning, organizing and implementation of care plans, and discharge planning. Collaborates with the Glucose Control Team, provides staff in-services and assists with the Diabetes Program, classes and outpatients as needed.
-Minimum Bachelor's degree in healthcare related field such as Registered Dietitian, Registered Nurse, Registered Pharmacist, PA-C or ARNP.
-2-3 years’ experience, with an emphasis on Diabetes Management and Education.
-Certified Diabetes Educator or BC-ADM, or is eligible to become a CDE or BC-ADM within 12 months of hire.
