Position Description: This position is responsible for providing clinical leadership and expertise in the delivery of patient care for Cardiovascular Surgery Patients and Patients with Valve Disease. Responsibilities include clinical coordination of Cardiac Surgery Patient care from pre-hospital evaluation to post discharge follow up including the STS and related Data registries. In addition this position will assist in the coordination of the Valve clinic and will collaborate with the team in the assessment and treatment planning of patients with moderate to severe valve disease including, cardiac stress testing and implementation of treatment protocols. This position will also assist with the educational development of the staff related to Cardiac Surgery and Valve Disease, and ongoing evaluations and improvement of the patient care functions based on registry data and quality improvement data. The position will assist in maintaining hospital compliance with regulatory standards, benchmarking programs and performance improvement initiatives. This position will also assist with the case management of Cardiac Surgery and Patients with Valve Disease to maintain strict adherence to quality and cost controls.
Requires strong working knowledge of performance improvement tools and methodologies, and effective analytical, organizational, and communication skills with ability to direct projects and assist with team building.

Essential Functions and Responsibilities of the Job
Demonstrates advanced knowledge and skill in diagnosis, management, and evaluation of actual or potential needs of patients and families with Cardiovascular Disease
Oversees and reports on STS data registries
Case manages Cardiac Surgery and Valve Disease patients in collaboration with Cardiac Nurse navigators to maintain quality and cost controls
Directs staff members in gaining and retaining clinical knowledge and skills
Serves as a resource person to facilitate clinical problem-solving and decision-making skills of clinical staff caring for patients having Cardiac Surgery and/or Valve Disease.
Functions as a change agent to improve patient care
Assist management with the attainment of organizational goals
Demonstrates knowledge of current research and literature
Addresses the age specific competencies of the population served (see checklist)
Conducts audit functions of various types both documentation and observational, analyze and present data
Experience 5 years ICU open heart experience required.
NP experience in acute care setting highly preferred, will consider strong school clinical experience in lieu. Experience with Data Registries and Quality Improvement tools. Strength in quality improvement theory, change theory, adult learning principles, and team building. Knowledgeable of current research and strength in translating research into everyday practice.
Effective problem solving skills. Ability to relate to a variety of professionals in the invasive and non-invasive cardiovascular setting. Good interpersonal and communication skills.
